Exercise: Fingertip to thumb touches
This exercise is all about control of the thumb whilst trying to minimise flexion from your other fingers. Most stroke survivors find it very difficult to achieve โfractionationโ: the ability to move fingers independently.
It's an exercise that asks you to flex, touch and extend again, repeatedly. Itโs not easy. I used to try doing it simultaneously even when one had no movement in โ this, Iโm sure, helped me amongst other things, to regain pretty good action control.
Technique.
Figs. 1 โ 4. Bring your affected hand down to the book, table or any flat surface. Help it to get there if necessary, whilst passively opening up the fingers as much as possible (Fig. 1). Lay your wrist firmly on the book with the back of your hand touching the surface. If you have high tone, continue to help loosen the fingers from the palm with the good hand. Touch the thumb firstly to the index finger (shown in Fig. 3), then the middle finger, the fourth finger (shown in Fig. 4) and then the little finger. Then reverse the movement so that you touch all fingertips back to the thumb. Keep going back and forth if you can. If you donโt have sufficient movement to touch fingertip to thumb, use an assist from your good hand combined with maximum effort to move voluntarily. This is probably best done by reaching over your bad hand with your good handโs index finger and thumb and moving the thumb to the appropriate positions. Use both hands simultaneously too.

In order to bring about functional change, neural pathways for desired activities need to be strengthened. At a very basic level, this can be achieved through repetitions; facilitated by โsmartโ use of use of some augmentations and principles which one can โaddโ to movements to optimise their performance over time; always trying to work โon the edge of your current ability. (How to do do this is explained in the stroke rehab possibilities 'wheel diagramโ in Had a Stroke? Now What? book by
Tom Balchin). The more repetitions you can achieve in a shorter time, the better the brain can re-structure.
Always remember the skill you are trying to โre-learnโ should be something that is relevant and meaning for you.

So,So,What do you think links the 3 leaders: From February 4 to 11, 1945, President Franklin D. Roosevelt of the United States, Soviet Union Premier Joseph Stalin, and British Prime Minister Winston Churchill met near Yalta in Crimea to discuss how post-World War II (WWII) Europe should be organized. Within 2 decades of this conference, all 3 men had died.
President Roosevelt died 2 months after the Yalta Conference due to a hemorrhagic stroke.
Premier Stalin died 8 years later, also due to a hemorrhagic stroke.
Finally, Prime Minister Churchill died 20 years after the conference because of complications due to stroke.
At the time of Yalta, these 3 men were the leaders of the most powerful countries in the world. The subsequent deterioration of their health and eventual death had varying degrees of historical significance.
Churchill's illness forced him to resign as British prime minister, and the events that unfolded immediately after his resignation included Britain's mismanagement of the Egyptian Suez Crisis and also a period of mistrust with the United States.
Furthermore, Roosevelt was still president and Stalin was still premier at their times of passing, so their deaths carried huge political ramifications not only for their respective countries but also for international relations. The early death of Roosevelt, in particular, may have exacerbated post-WWII miscommunication between America and the Soviet Union-miscommunication that may have helped precipitate the Cold War.
